Borden County Clerk Information

The Clerk processes birth and death certificate requests. They have provided an application form for these requests, and is linked below. The form requires the name of the owner of the certificate, parents’ name, date of birth or death, as well as contact information of the applicant to be able to be contacted back. together with the application, you also need to mail in the sworn statement (included in the document below), payment, and a photocopy of a valid ID.
