A vital record information and application form is available for download in their website. Only certified copies will be provided to authorized individuals; and must be retained by mail or personal pickup. Photo ID is required for birth or death vital records (and, at the discretion of the County Clerk’s Office, for some Marriage Records). If you do not have a photo ID, then a copy of your Social Security card. You need to use a separate application for each record requested, and for mailing purposes, a self-addressed, stamped envelope is required.

Court Clerk Information
The Clerk is required to attend all sessions of Court, keep and preserve all court records, issue process as required by law, and are the Keeper of the Seal of the Court. On a daily basis, the office interacts with the public and attorneys performing several functions including the collection and disbursement of fines, civil fees, and child support. It is also the duty of the Circuit Clerk to summon prospective jurors for both petit and grand jury duty.
